Date when you wish to have your child first attend CIS :  
Term One starts at the begining of September. The Second Term starts at the begining of February.
This are the normal entry points to our programs. Other times will be considered depending on space.
Field Trips : Do you agree to allow your child to participate in field trips in the local area ? Yes No
Note: Parents will be notified separately for every field trip that is planned but not further permission will be requested. We do recognize the right of parents to refuse to allow their child to participate on a particular field trip for cause. Please inform the school in writing if you wish to have your child excused from a particular field trip.
Internet Access :
As part of the program, students are expected to do supervised research on the internet. We have pre-selected sites which may be used. Do you agree to allow your child to use the internet while at CIS ? Yes No

Terms and Conditions to be understood before acceptance at CIS. Students will not be guaranteed a placement unless Tuition Fees are paid in full. Student will not be allowed to attend classes if Tuition, Textbook, or Misc. Fees are not paid in full by the due date. Not refund on the Tuition Fees after each School Term has started in September or in February each year.
